Family Wellness Counselor - Bilingual: Spanish

Job Overview The Family Wellness Counselor position is responsible for general overview of women’s health patients. The Family Wellness Counselor provides interviewing and assessment for all women’s health patients, counsels those individuals who are identified as benefitting from time limited, symptom focused psychotherapy or social supports, triage individuals who have complex needs, refer patients to specialty programs, communicate with outside agencies regarding referral and follow-up of patients, provide crisis support and triage patients across the organization as deemed appropriate. Requires strong interpersonal communication skills and knowledge of community resources for a range of medical and social services, as well as, skills related to both diagnostic interviewing and psychotherapy. ESSENTIAL DUTIES & RESPONSIBLITIES Team Focused Provision of Care Works as a part of an assigned care team, defined as – provider, medical assistant, nurse, referral coordinator, with the option of including care manager, nutritionist and behavioral health provider, as indicated. Team based communication is an expectation and should include interaction with support staff (MOAs, Call Center, enabling service providers) to encourage the engagement of the patient as a part of the care team. Electronic Medical Records & Computer Responsibilities: * Responsible for the daily utilization of the Electronic Medical Record. * Review of patient specific billing and diagnostic codes within the Electronic Medical Record. * Initiate communication and/or follow-up using the EMR patient case system to Clinician, Referral Coordinators and other Support Staff. * Required to attend trainings and workshops to refresh and improve knowledge of the EMR. * Update Excel spreadsheets with data on prenatal patient information and UDS tracking. * Competency of basic computer skills, to include but not limited to Microsoft Outlook. * Ongoing daily monitoring of Microsoft Outlook and Microsoft TEAMS. Essential Duties: * Performs Family Wellness Counselor duties in compliance with all regulatory and health center guidelines. * Interviews perinatal clients (both scheduled and walk-in), completes psychosocial assessment and develops a treatment plan in conjunction with the patient. Meets with patients for subsequent appointments, including postpartum visits. Document known follow through on referrals * Facilitates scheduling of patients for appropriate on-site services (including midwifery services, GYN, C&T, adolescent assessment). Collaborate with clinicians and other providers on an ongoing basis to discuss case progress. * Arranges meetings with all those involved with treatment plan (midwives, physicians, partners, parents or another responsible adult for pregnant adolescents) to reinforce and aid in treatment plan adherence. * Refers patients to other resources both within and outside of the community when the treatment plan is beyond Settlement Health’s resources. Collects and maintains information on agencies to which patients have been referred. * Provides advocacy to patients who have identified obstacles in obtaining other services. * Provides patients with short-term individual therapy, with Supervisor’s approval. * Create, plan, and facilitate clinical and psychoeducational support groups and workshops as needed. * Attend internal and external trainings for improvement on clinical modalities as needed. * Participate in weekly crisis coverage rotation providing crisis assessment, intervention, and safety planning when providers deem it necessary. * Provides counseling about birth control choices, including detailed Tubal Sterilization counseling and obtaining consent for prenatal patients. * Keeps close records of Family Wellness Counselor activities. Achieves contract/program specific performance goals. * Participates in appropriate department and health center meetings and committees as needed. * Performs other programmatic duties as requested by supervisor, Director of Integrated Clinical Care. * Represents Settlement Health at community functions and committee meetings when assigned by Supervisor and reports activity to staff at department meetings. * Meet with supervisor for weekly individual clinical supervision and group supervision as needed. * May be required to perform duties and responsibilities not listed in this description on a temporary or long-term basis. QUALIFICATIONS * EDUCATION/EXPERIENCE o Masters Degree in Social Work, Counseling or related field is required o Experience working with prenatal patients in a primary care setting preferred * SKILLS AND KNOWLEDGE o Fluency in English and Spanish required and the ability to speak in a professional and courteous manner o Possess strong customer service skills, a positive attitude and the ability to be proactive, resourceful and flexible o Basic computer skills with windows-based programs and internet navigation and experience with Electronic Health Records * AVAILABILITY REQUIREMENTS o Can commit to a schedule of 9am - 5pm - Mon-Fri o Ability to work 1 Saturday a month as part of Behavioral Health team rotation. o Hybrid work model: 1 Remote day per week per discretion of supervisor. * CERTIFICATION, LICENSES, REGISTRATIONS o LMSW, LCSW or LMHC active and issued by the state of New York. o Clinical hours provided through clinical supervision if needed towards LCSW licensure for social workers.
