Prescription Coordinator Thresholds Clinic Tapestry 360 Health Tapestry 360 Health is committed to providing comprehensive and compassionate care to underserved communities across Chicago. As a Prescription Coordinator, you will play a critical role in ensuring patients have timely and affordable access to medications and durable medical equipment (DME). By coordinating medication refills, obtaining prior authorizations, and supporting the 340B drug pricing program, you will directly contribute to improving patient outcomes in a dynamic, team-based environment. Essential Duties and Responsibilities: * Prescription Management: Communicate with healthcare providers, pharmacies, and patients to facilitate timely processing of medication refills and DME requests. * 340B Program Support: Assist patients in accessing 340B pricing to ensure affordability of medications, and ensure compliance with 340B program guidelines. * Prior Authorizations: Manage the process of obtaining prior authorizations for high-cost medications, ensuring all documentation is accurately maintained in the EMR system. * Medication Shortages: Collaborate with providers to identify alternative medications during shortages and ensure availability of appropriate treatments. * Clozapine REMS Management: Track and report lab results for patients prescribed high-risk medications, ensuring compliance with Clozapine REMS requirements. * Documentation: Maintain thorough and accurate records of all prescription and DME transactions in the electronic medical records system. Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. * Required Education: Completion of an American Society of Health-System Pharmacists (ASHP) approved pharmacy technician vocational course. * Required Experience: At least one year of experience in a hospital or retail pharmacy setting. * Certification: Current Licensed Pharmacy Technician certification in the state of Illinois. * Preferred: Bilingual and/or bicultural, with experience in an FQHC or community health setting. Skills and Competencies: * Technical Skills: Proficiency in Microsoft Office (Excel, Word, Outlook, PowerPoint) and electronic medical records (EMR) systems. * Communication: Strong interpersonal and written communication skills, with the ability to effectively collaborate across teams and with diverse patient populations. * Problem Solving: Demonstrated critical thinking and problem-solving abilities, with a proactive approach to managing medication-related challenges. * Empathy and Teamwork: Ability to build respectful relationships with patients and team members, fostering a collaborative and supportive work environment. Benefits: Tapestry 360 Health offers a comprehensive benefits package including health insurance, dental insurance, retirement savings plans, paid time off, continuing education support and eligibility for the Public Service Loan Forgiveness (PSLF) program. Pay Transparency: The salary range for this position is $48,000 - $60,000 annually. Actual compensation will be determined based on factors including, but not limited to, relevant experience, qualifications, internal equity, and organizational needs.
