> Position: Clinical Pharmacist Practitioner Location: Minnie Jones Health Center 257 Biltmore Avenue Asheville, NC Job Id: 813 # of Openings: 1 Clinical Pharmacist Practitioner- Asheville, NC Are you passionate about helping the underserved? Western North Carolina Community Health Services (WNCCHS) is a Federally Qualified Health Center (FQHC). We are looking for a Clinical Pharmacist Practitioner (CPP) that has a passion for serving the underserved and individuals who need higher levels of service in a high-touch, fast-paced environment. The CPP will be responsible for providing primary care-focused pharmacotherapy services within the clinic and evaluate and manage patients with chronic conditions (e.g., DM, Hyperlipidemia, Hypertension, COPD/Asthma, Congestive Heart Failure, Smoking Cessation) through direct patient care as outlined in the individual's collaborative practice agreement. The specialist will work closely within a multi-disciplinary team of primary care physicians, advanced practice clinicians, medical residents, nurses, and pharmacy staff to optimize and promote rational, safe, evidence-based, and cost-effective therapy. The CPP is responsible for working with patients to identify strengths and barriers and to develop an individualized, patient-centered pharmacotherapy care plan. This in-person position is located at our Minnie Jones Health Center, 257 Biltmore Avenue, Asheville, NC 28803. QUALIFICATIONS: Education: * Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Certification/Licensure: * Full, current, and unrestricted license to practice pharmacy in North Carolina from the North Carolina Board of Pharmacy. Experience: A CPP, as defined, in 21 NCAC 46.3101, you must be a licensed North Carolina pharmacist and have an agreement (collaborative practice agreement) with a supervising physician, as defined in 21 NCAC 46.3101 (6). In addition, you must have either: * Completed a Board of Pharmaceutical Specialties (BPS) Certification or Geriatric Certification, or the American Society of HealthSystems Pharmacists (ASHP) accredited residency program and have 2 years clinical experience, OR * Earned a PharmD degree, have 3 years' experience, and have completed a Certificate Program, OR * Earned a BS degree, have 5 years' experience, and have completed two certificate programs.
