Are you living your life's work?Entity: Penn Primary Care and Penn Specialty Practices Department: Population Health Location: 150 Monument Road Hours: Full-Time, Per Departmental Needs Job Summary: This position supports the entity in performing the project management, reporting and data collection & dissemination duties essential to improving practice performance on quality and population health measures. Accountabilities: Responsible for reporting, data collection and information distribution on projects integral to sustaining quality improvement (QI) initiatives, and payer programs related to population health efforts.Manages disease-based dashboards and registriesIdentifies care gaps in collaboration with appropriate clinical leadershipCoordinate strategic efforts in addressing the population health care gapsWorks with data analyst to run reports on chronic disease management metrics and utilizationWorks with data analyst to maintain and develop monthly reports of key elements needed for the projects and feedback to the practice on progressCoordinate process improvement projects and provide implementation support to care teamsProvide project management support for the departmentReviews and completes reports for city, state and federal agencies, including payers, as directed.Completes and submits monthly reportsPerforms duties in accordance with Penn Medicine and entity values, policies, and proceduresOther duties as assigned to support the unit, department, entity, and health system organizationQualifications: Bachelor's Degree (Required)2+ years of experience in Healthcare (Required)Master's Degree in Public Health (Preferred)We believe that the best care for our patients starts with the best care for our employees.
