Position Summary: This individual will provide behavioral health support to residents of long term care facilities under the supervision of a licensed professional. They may receive training in a broad range of assessments and interventions that will support treatment specified in individual care plans. Regional travel will be required. Essential Duties and Responsibilities: The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive, other duties may be assigned:Conduct assessments that include the behavioral and social needs of the patientsMeet with families and facility staff to coordinate care as recommended by licensed professionalsAssess the patient's knowledge of their clinical condition and provide education and self-management supportProvide outreach to clients who are having problems with treatment adherenceAssist in the development of the individual goals and behavioral health treatment planCoordinate with other agencies and community providersMaintain timely documentation of workServe as an active member of the behavioral health treatment teamEducation and/or Certifications:Bachelor's degree in Counseling, Social Work, Psychology or related fieldSkills and Qualifications:Experience in a psychiatric or long term care setting preferredCustomer service experience preferredAbility and willingness to function independently and proactively in a long term care settingEffective use of electronic medical records and Microsoft software (teams, excel spreadsheets, etc)Skilled in written and verbal communication.Skilled in exercising a high degree of initiative, judgment and discretion.Ability to handle confidential and sensitive information.Physical Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the job.While performing the duties of this job, the employee is regularly required to communicate with others, including the ability to express oneself and exchange information in what, at times, could be a stressful or high-pressure environment.The employee frequently is required to move about the office to access files, use office equipment, and to interact with others. The person must be able to sit or remain in a stationary position for extended periods of time to be able to work, communicate on the phone, utilize the computer, and interact with others. The employee is occasionally required to stand or otherwise be able to access files, equipment, and other items in high places.
