RTO must be on the cover page and cannot exceed 7 days. No RTO during the first week of assignment. Candidates must not have hits on their state license to practice. Flu vaccination status should be included in submission notes. Medical and/or religious exemption forms for flu vaccination are available upon request. Travel experience is highly preferred. Description: Candidates must meet all requirements outlined. The cover page needs to be 100% accurate with all requests noted.
