Salary: $80,000.00 AnnuallyLocation : Williamsburg, VAJob Type: Full TimeJob Number: 105519Department: Integrated CareOpening Date: 03/05/2026 Job DescriptionColonial Behavioral Health provides opportunities for recovery to individuals and their family members in the areas of mental illness, substance use disorder, and developmental disabilities. Colonial Behavioral Health serves the citizens of James City County, City of Poquoson, City of Williamsburg, and York County.We are currently recruiting a licensed provider for this innovative, high-impact position focused on developing and implementing a new integrated behavioral health consultation model in close partnership with multiple internal and external providers. The Behavioral Health Consultant will play a key role in designing workflows, testing and refining new processes, supporting change management across partner sites, and using data and outcome measures to inform continuous improvement. This role requires strong initiative, planning and problem-solving skills, flexibility in a rapidly evolving environment, and the ability to adapt services based on operational realities and partner feedback. The work will serve as a proof of concept for CBH's future integrated care model and will ultimately transition into a newly designed facility on CBH's new campus, currently in the planning and design phase.Job Duties Job ResponsibilitiesProvide comprehensive assessment and diagnosis of behavioral health clients.Provide direct care including assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders.Provide effective treatment planning and assist clients in successful referrals for ongoing long-term intervention if needed. See patients for a limited time and a limited number of visits as needed.Evaluate crisis situations and apply appropriate interventions.Actively participate in meetings that support integrated healthcare models to provide comprehensive care for clients.Assist in detecting "at risk" patients and developing plans to prevent further psychological or physical deterioration.Assist the primary care team in developing care management processes such as using guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.Teach patients, families, and staff care, prevention, and treatment enhancement techniques. Maintain a schedule of a limited number of repeat patients while also being available same day for warm handoffs from providers.Monitor the progress of the integrated health care model, identifying problems related to patient services and making recommendations for improvement.Reports summary data to Integrated Care team at least quarterly and leads discussion of analysis. Qualifications (EEO) Skills and QualificationsAbility working independently and collaboratively with an interdisciplinary team.Must have strong oral and written communication skills, excellent organizational and record-keeping skills, and strong research, problem-solving, and decision-making skills.Must be able to prepare a comprehensive recovery-oriented treatment plan according to agency guidelines.Ability to maintain an objective, professional and calm relationship with individuals and groups in a stressful situation At least one year of work experience with individuals with serious mental health and/or developmental disabilities required. Valid driver's license is required.
