Job Description We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to provide direct patient care in community-based and non-traditional settings, with a strong focus on serving individuals experiencing homelessness and other underserved populations. This role is ideal for someone with hands-on experience in wound care, outreach, and building trust with patients in complex environments. The LVN will work closely with a multidisciplinary team to deliver high-quality care, support coordination of services, and promote overall health and wellness. Responsibilities/ Job Duties: Provide direct patient care under the supervision of a licensed physician or registered nurse in field-based, mobile, and community settings Perform wound care treatments, including assessment, cleaning, dressing changes, and monitoring for infection or complications Conduct health assessments, obtain vital signs, and document patient information accurately in EHR systems Administer medications and treatments in accordance with clinical protocols Deliver care to individuals experiencing homelessness, using a trauma-informed and patient-centered approach Build rapport and establish trust with patients in non-traditional and high-acuity environments Provide bilingual (Spanish/English) education on medication adherence, chronic disease management, and preventative care Collaborate with interdisciplinary teams (providers, case managers, social workers) to develop and implement care plans Assist with crisis intervention and de-escalation in challenging situations Participate in outreach efforts, including engaging patients in encampments, shelters, and community locations Coordinate referrals to housing resources, mental health services, and community-based programs Maintain infection control standards and ensure safe care practices in field settings Track and maintain inventory of medical supplies and mobile equipment Minimum Education and Experience Qualification Requirements: Active and unrestricted LVN license in the state of California Bilingual in Spanish (required) Minimum 1-2 years of clinical experience, preferably in: Community health Street medicine or homeless services Urgent care or public health settings Strong experience in wound care and treatment management Current BLS/CPR certification Comfortable working in outdoor, mobile, and non-traditional healthcare environments Valid driver's license and reliable transportation Schedule/ Shift: Monday - Friday: 7:30/8:00am - 5:00pm * We will consider for employment all qualified Applicants, including those with criminal histories, in a manner consistent with the requirements of applicable federal, state, and local laws, including the City of Los Angeles' Fair Chance Initiative for Hiring Ordinance (FCIHO), Los Angeles Fair Chance Ordinance for Employers (ULAC), The San Francisco Fair Chance Ordinance (FCO), and the California Fair Chance Act (CFCA). * As a job position within our Allied division, a successful completion of a background check may be required as a condition of employment. This requirement is directly related to essential job functions including but not limited to: providing direct patient care, accessing medical and confidential records, accessing and administering prescription medication or other drugs, working within a clinical setting, handling sharp instruments, conducting medical procedures, and working within departments that care for vulnerable populations, such as, minors, elderly and those with physical or mental disabilities.
