Salary Range: $79,097.20-$100,318.40Night Shift differential availableThe above salary range represents a general guideline; however, OneLegacy considers a number of factors when determining base salary offers such as the scope and responsibilities of the position and the candidate's experience, education, skills and current market conditions.Work Setting: In-person and Field Service AreaLocation: Employee may assigned to a base office in El Segundo Travel: Travel required by personal auto or air to meeting sites and other locationsSummary of Functions:The Donation Clinical Coordinator (DCC) has high proficiency in the referral management process and represents OneLegacy by reinforcing and supporting donation related processes as outlined in the UAGA (Uniform Anatomical Gift Act) and donation specific laws of the California Health and Safety Code.The DCC is responsible for assisting hospitals to identify and refer potential donors, gathering and interpreting medical information to aid in the determination of suitability for a potential donor, discussing the plan of care with the medical team, providing donor management goals to help preserve the opportunity for donation, facilitating completion and review of brain death notes, representing OneLegacy to the potential donor's authorizing party and when appropriate, undertakes registry notification or approach for donation authorization when necessary, as well as assisting in the donation process occurring at a OneLegacy Recovery Center including, but not limited to, transportation, logistics, documentation, donor care, and organ recovery when necessary.Duties & Responsibilities:Essential Job Functions:Referral Management Process:1. Responds promptly and appropriately to potential donor referrals to assess suitability and enhance the donation process.2. Establishes and enhances relationships with the hospital staff to increase referral activity.3. Introduces themselves to the referred patient's bedside nurse and the unit Charge Nurse to establish presence. When leaving, notifies the bedside Nurse and Charge Nurse, discusses plans and assessment, and inquires if there is anything else needed. The attending physician should be knowledgeable of the OneLegacy referral with direct communication at least for the initial site visit and prior to any approach to the family or Authorizing Party (AP).4. Collaborates with the Referral Triage Specialist (RTS), Referral-Supervisor of Organ Procurement (SOP-R) and/or the Medical Director of Referral Management to manage all active referrals.5. Accesses and reviews the referring hospital's medical records and populates the Electronic Medical Records system per OneLegacy policy.6. Reviews the patient's medical records and discusses the early stages of medical derangements, current organ function and the current clinical plan with the hospital staff. Based on any abnormal findings, formulates a treatment plan at the direction of the Medical Director of Referral Management and collaborates with hospital staff and physicians regarding interventions needed to optimize organ function and preserve the opportunity for donation.7. Communicates effectively with hospital staff regarding the progression of active referrals.8. Reviews medico-legal documentation pertaining to brain death declaration for completeness and accuracy according to individual hospital policy, California Health and Safety code and the 2010 American Association of Neurology Guidelines. When revisions are needed, provides clear direction to appropriately complete documentation. Sends acceptable brain death documentation to the RTS for verification per OneLegacy policy.9. Performs the function of Referral Intake Triage (RIT) which includes being responsible for answering calls for initial organ referrals, triaging referrals, and charting in the Electronic Medical Records system in collaboration with the RTS to establish acuity.10. Reviews the Not Brain Dead (NBD) status board when in a hospital for an active referral.11. Conducts a site visit on NBD referrals and charts in the EMR system.12. Identifies the Authorizing Party (AP) and any language requirements. Remains vigilant and documents pertinent family dynamics and any other relevant information needed to adequately assess the AP and/or family members of potential donors in preparation for an approach.13. Collaborates with the hospital care team and internal OneLegacy partners in the end-of-life discussion to ensure the AP is provided with the opportunity for donation in the most appropriate manner.14. Works with OneLegacy Clinical Donor Management team members during the donation process to facilitate orders to the bedside Nurse or Physician providing care to the donor.15. Performs the following:a. Coroner notification/release.b. Requests that an initial blood sample in the lab is "on hold" for the coroner, height and weight verification, plasma dilution calculation, generate requisitions for serologies and HLA, labeling/packaging & shipping of blood for HLA & serological testing while charting in the Electronic Medical Records (EMR) system.Referral Management Precepting:1. DCC functions as primary trainer for new hire RCCs under the direction of the Referral Management Leadership and Education teams.2. Collaborates with Referral Management Leadership to monitor and maintain department staff competencies and training logs/forms. Will also utilize multiple training tools provided by the Leadership and Education teams.3. Will participate in "Train the Trainer" meetings to update/discuss staff currently training and help develop plans for staff to get signed off in the future.Donation after Cardiac Death (DCD) Donor Management:1. Discusses and supports the donation process with family members or Authorizing/Acknowledging Party (AP).2. Manages the administration of the DCD order set to start the case after authorization.3. Updates donor vitals and lab results, in line and in pace with the progression of the donor in collaboration with clinical and logistics OneLegacy team members.4. Works with appropriate hospital personnel to order all applicable diagnostic testing to assess viability and suitability of potential organs for transplant.5. Performs upload of donor related documentation to digitalDonor and DonorNet as appropriate to progress the donor toward organ allocation.6. Works with the Donor Allocation department to ensure all needed and necessary documentation, lab values, test results, diagnostics are complete and accurate for the purposes of organ allocation.7. Collaborates with the Medical Examiner/Coroner, tissue recovery staff, hospital staff, social services, and clergy.8. Serves as the point of contact for transplant center personnel when needed to discuss current donor stability and clinical condition.9. Works with appropriate hospital personnel to secure services to facilitate withdrawal of life support for the purposes of organ recovery in the event the donor expires within a time frame compatible with the donation.a. Secures pronouncing MD or equivalent.b. Secures Operating Room time, or equivalent for terminal extubation.10. Supports organ donor family members in person and telephonically, providing updates on the donation process timelines and other needs and considerations.Other Responsibilities:1. Maintains competency for the position through annual assessments outlined by the Medical Director of Referral Management.2. Participates in regularly scheduled staff meetings and/or conference calls.3. Observes a code of ethics that requires truthfulness, honesty, and personal integrity in all activities. An employee's professional life should be conducted in a manner that would reflect credit on the organization. Employees should present themselves at all times in a manner that would represent professionalism, respect, and dignity for the gift of donation.4. Performs other duties as assigned.5. Maintain current ACLS/PALS certification.6. Maintenance of EMR access and hospital accounts.Physical Environment/Working Conditions:Location: OneLegacy's Donor Service Area (DSA)Travel: Travel required by personal auto or air to meeting sites and other locationsWork Hours: Assigned days and/or shift can be changed according to staffing needs. Must be available to work evenings, holidays, and weekends and overtime as required or assigned. The length of scheduled shift may be extended in order to accommodate fluctuation in referral activity and staffing needs.Job Qualifications:Education: LVN with critical care experience or prior relevant OPO experience preferred. Associates or Bachelor's degree preferred. Knowledge of Microsoft and Android applications required.Experience: Required previous hospital employment, experience in the medical field, 911, and/or ICU experience is preferred. Medical terminology is preferred.Certification/License: Basic Cardiac Life Support (BCLS), Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certifications are required within 6 months of hire. OneLegacy requires employees to maintain a current California driver's license and current vehicle insurance.
