We empower people to break the cycles of homelessness, welfare dependency, substance abuse, and incarceration through innovative work and housing programs.We are seeking a Case Manager/Mental Health Counselor for The Doe Fund's Webster permanent supportive housing program for homeless men and women living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. Half of the residents will be referred by the New York City HIV/AIDS Services Administration (HASA), and the other half will be apartments for affordable to low-income households.The Case Manager/Mental Health Counselor is responsible for providing supportive case management services to the HASA-funded residents. The successful candidate will have strong clinical and communication skills along with an understanding of working with the HIV/AIDS population.We offer a competitive salary with comprehensive benefits, including medical and dental coverage, paid participation in a retirement plan, sick and vacation leave, discount gym memberships, paid holidays and more. The Ideal Candidate:You have a Master's Degree in Social Work or related field, a license in Clinical Social Work and LMSW or LMHC required. You have at least one year of experience working with people who are homeless, living with HIV/AIDS, have substance abuse histories, or suffer from mental illness You have at least 5 years of experience in the mental health fieldYou have knowledge of benefits in the New York City Metro area, HIV/AIDS, psychiatric diagnoses, substance abuse, and mental health issuesYou have solid computer skills - internet, Microsoft Office, and charting database You have great organizational, writing, and communication skillsKey Responsibilities:Maintain a caseload and provide regular case management services to these individualsMaintain resident records in compliance with agency and funding standards Develop and implement individual Service Plans and assist residents with achieving their goals geared towards self sufficiency Monitor and ensure residents are linked to healthcare providersProvide counseling and crisis intervention as necessaryAssist residents in securing and maintaining benefitsMaintain a liaison with community services including hospitals, substance abuse treatment programs, HRA and mental health programsFacilitate groups and activities that promote health and independent living skills developmentMonitor and assist residents with rent payments and budgetingConduct regular unit inspections Educate residents about approaches to mental health Attend ongoing trainings to enhance the services provided to the residentsOther duties as assignedInterested?Submit your application for review.
