Location: Detroit, MI and surrounding areasSchedule: Full-Time Preferred Monday Friday, 8:00 AM 4:30 PMNo weekends, no holidays, no on-call About the Role We are seeking a highly skilled and compassionate Wound Care Nurse Practitioner to provide advanced, patient-centered care for individuals with acute and chronic wounds across home settings, assisted living facilities, and skilled nursing facilities. This is a mobile, community-based role combining in-person visits with telehealth support, allowing you to deliver impactful care where patients need it most.This opportunity is ideal for a provider who thrives in an autonomous environment, enjoys working with complex cases, and wants to be part of an innovative, growth-focused care model. Key Responsibilities Provide in-person evaluation and management of patients with acute and chronic wounds and skin disorders in home, ALF, and SNF settingsPerform medically necessary procedures, including debridement, in compliance with CMS guidelinesDeliver virtual care and follow-ups through telehealth and remote monitoringDevelop and implement evidence-based treatment plans to optimize healing outcomesAccurately document patient encounters and complete billing documentation within 1 business dayCollaborate with interdisciplinary teams including home health, primary care, and specialistsCoordinate ongoing care and ensure seamless communication across providersMaintain strict adherence to infection control and HIPAA compliance standardsParticipate in quality improvement initiatives and program developmentStay current with best practices through continuing education and certification maintenance Patient Population & Workflow Adult patient population (approximately 90% geriatric)High-acuity cases, including stage 3 & 4 woundsDaily patient volume varies based on travel and complexity:2 3 patients/day (complex or spread out cases)4 6 patients/day (clustered locations)Documentation completed remotely (iPad and laptop provided)All supplies shipped directly no clinic or brick-and-mortar site Training & Onboarding Initial in-person training in Minneapolis (Mon Thurs, all expenses paid)Followed by virtual shadowing and ramp-up periodExpected to practice more independently by approximately week 6 Qualifications Required:Master s Degree in NursingActive RN and NP licensure in practicing state(s)Board Certification as a Nurse PractitionerCurrent Wound Care CertificationMinimum:3+ years experience as a wound care specialist (acute care, SNF, home health, or outpatient)1+ year as an NP in a wound care roleValid driver s license, reliable vehicle, and active auto insuranceComfortable traveling to patient homes and facilitiesPreferred:WOCN-accredited training and WOCNCB certification (CWCN)Experience in home health or autonomous practice settingsFamiliarity with Athena EMRHospital or outpatient wound care experience Compensation & Benefits Salary: $122,000 $158,000 annuallyMileage reimbursement at federal rate100% employer-paid medical insurance (employee)75% dependent coverage4 weeks PTO accrual annually8 paid holidays + 4 floating holidays401(k) planCompany-provided equipment (iPad & laptop)Flexible scheduling preferences
