General: This position, under the supervision of the Director will assist the program staff with case management administrative duties and client record keeping. Job Requirements: * Maintaining client records in accordance with established policies and procedures * Coordinating, monitoring & documenting relevant issues for clients * Providing technical support for program staff, including client intake * Serving as a liaison between the professional staff, community agencies and clients * Participating in all mandated staff development sessions * Assisting with community resource meetings * Facilitate transportation for clients * Monitor utilization of program resources * Contacting institutions to confirm resident departure times * Generating program reports to track program services * Tracking client participation * Performing other duties as assigned by Program Personnel Qualifications/Skill Requirements: * One year of related experience in an office environment. Abilities: * Understand and comply with company and contract agency rules, regulations, policies and procedures; * Communicate effectively and all levels within and outside the organization; * Express ideas clearly and completely, written and oral; Manage time to meet the responsibilities and duties of this position; * Give attention to detail and follow-through; * Establish and maintain appropriate working relationships with co-workers and contracting agency representatives; * Execute all duties in accordance with this position description. Education requirements: * High School or Equivalent
