Travel Required: Yes, routine travel between assigned Texas skilled nursing facilities. Schedule: Full‑time role, primarily onsite at partner facilities with routine travel between locations. This full‑time, travel‑required role focuses on provider relations, member experience, care coordination support, and facility liaison responsibilities. The Liaison conducts daily onsite visits, builds strong relationships with SNF leadership, supports prospective members and families, provides plan education, and collaborates with integrated care teams to resolve issues quickly and professionally. This position requires strong communication skills, independence, and the ability to represent ProCare Advantage across multiple Texas markets.
