This is an excellent opportunity for a nurse leader who enjoys variety, values autonomy, and is passionate about delivering exceptional home-based patient care while supporting and mentoring clinical staff across the region.Job specificationCandidate requirementsJob specification• Provide regional coverage as a Patient Care Manager across multiple home health locations as needed. • Oversee the development, coordination, and implementation of individualized patient care plans. • Ensure high-quality, compliant patient care delivery in accordance with Medicare, state, and regulatory guidelines. • Supervise, support, and collaborate with clinical staff to promote best practices and optimal patient outcomes. • Review, monitor, and ensure accuracy and timeliness of OASIS documentation. • Serve as a clinical resource and leader, assisting teams with complex patient care situations. • Collaborate with physicians, therapists, and interdisciplinary team members to coordinate care services. • Monitor patient progress, outcomes, and compliance with care standards across assigned locations. • Support quality improvement initiatives and ensure adherence to organizational policies and procedures.
