Overview A well-established senior care community in Freehold, NJ is seeking a compassionate and experienced Nurse Practitioner to provide high-quality, patient-centered care to a geriatric population. This role spans primary care, subacute, long-term care, and assisted living settings, offering a dynamic and meaningful clinical environment.This is an excellent opportunity for an NP who thrives in a collaborative yet autonomous role, as you will serve as the sole provider onsite during scheduled days while working closely with an interdisciplinary care team. Schedule Monday, Wednesday, Friday 8:30 AM 5:00 PMOption to add Tuesday or Thursday at an additional nearby locationFlexibility for part-time or expanded full-time schedule Key Responsibilities Conduct comprehensive assessments and physical exams for senior residentsDiagnose and manage acute and chronic conditionsDevelop and oversee individualized care plansPrescribe medications and treatments as appropriateProvide education and counseling to patients and familiesCollaborate with physicians, nurses, and interdisciplinary teamsMaintain accurate and timely documentation in EMRParticipate in care team meetings and quality initiativesSupport efforts to reduce hospitalizations and readmissionsPerform IV insertion as needed (infrequent) Patient Population & Workflow Geriatric patients with complex, multi-morbidity conditionsMix of subacute, long-term care, assisted living, and primary care visitsAverage patients per day ( 2 3 per hour)Smaller, close-knit setting with 200 residentsStrong emphasis on patient experience, bedside manner, and family communication Required Qualifications Master s Degree in Nursing (MSN) from an accredited programActive New Jersey Nurse Practitioner license 5+ years of experience in primary care or geriatricsRecent (within 3 years) subacute or acute care experienceStrong clinical judgment and knowledge of geriatric careAbility to work independently and collaborativelyExcellent communication and interpersonal skills Preferred Qualifications FNP or AGPCNP certificationExperience in long-term care or medical group settingsWillingness to travel between nearby locations (mileage reimbursed)Flexible, team-oriented mindset What Makes This Opportunity Stand Out Autonomy + Team Support Lead care onsite while backed by an interdisciplinary teamMeaningful Patient Impact Focus on continuity of care and reducing hospitalizationsFlexible Scheduling Build a schedule that fits your lifestyleClose-Knit Environment Smaller patient panel with strong community feelHigh-Touch Care Model Emphasis on relationships, not just volume
