We are seeking a highly skilled Family Practice Nurse Practitioner to provide compassionate, comprehensive care through telehealth and in-home visits in Jefferson County, Franklin County, and St. Louis County. Position Overview: As a Family Practice Nurse Practitioner, you will deliver vital healthcare services that include annual risk assessments and chronic condition reviews for both pediatric and adult patients. The role is assessment-focused, with no treatment or prescribing required, and you will be integral in managing patient follow-ups and escalating urgent findings to primary care providers. Expect a balanced mix of telehealth (70%) and home health visits (30%), scheduled within a 30-mile radius of the provided address. Your workday will typically include 12-14 telehealth visits or 6-7 home visits, with an average of 10 in a balanced schedule. Equipment will be mailed and provided, ensuring you have the tools needed to succeed. Required Skills: Valid Medical License in MissouriProven experience in family or primary careComfort with pediatric and adult patient populationsStrong assessment and communication skillsProficiency with telehealth technology and Electronic Medical Records (EMR)Ability to work independently and adhere to scheduled appointmentsExcellent organizational skills and attention to detailNice to Have Skills: Experience with home health care servicesFamiliarity with Salesforce or similar scheduling platformsBilingual abilities in English and SpanishPrior telehealth clinical experiencePreferred Education and Experience: Master s Degree in Nursing (MSN) or higherAt least 2 years of clinical experience as a Nurse Practitioner in a primary care settingCertification as a Family Nurse Practitioner (FNP-C or FNP-BC)Other Requirements: Enrollment and credentialing in Medicard (please confirm and highlight in your application)Ability to commit to a start date of May 18, 2026Flexibility to work 40 hours per week, Monday through Friday, 8:00 am to 5:00 pm, with a 1-hour lunchWillingness to complete ongoing education modules during working hoursParticipation in approximately 1-3 hours of paid orientationThis position offers an hourly pay rate of $95.48, paid weekly via direct deposit.
