This is a meaningful field role focused on caring for patients where they are most comfortable at home during some of the most important moments in their recovery.You will manage an assigned panel of patients receiving acute and transitional care in the home, including individuals recently discharged from the hospital, emergency department, or skilled nursing facilities, as well as medically complex, homebound patients with lower acuity needs.If you value relationship-based care, clinical autonomy, and the opportunity to truly make an impact, this role offers a rewarding alternative to traditional clinic or facility-based practice.
