Our mission is to provide the highest quality of care for our patient/clients by matching people with meaningful, long-lasting, and enjoyable careers, not just a job.Responsibilities:Monitor patient's condition and report changes/concerns to appropriate medical professionals and family membersProvide Activities of Daily Living care as outlined in the Care Plan-Meal Prep, toilet assist etc.Transfers patient with equipment using appropriate safety measures - pivot assist or hoyer-lift. (Not all clients need this)Perform light housekeeping duties to maintain a clean-living space for clients.Understands First Aid and Emergency Care protocolsComplete electronic documentation while on shiftSkills:Familiarity with Home Health Care, safety and sanitation standards and proceduresUnderstanding of Confidentiality and Privacy LawsExcellent communication and interpersonal skills… able to communicate effectively with patients and their families.Knowledge of basic medical terminologyDetailed orientedAbility to undertake a variety of diverse care tasks and work as a team memberCompassionate and able to handle tough situations as needed1 year experience as a nurse/CNA/caregiverDementia care experienceCarePlus TLC is a locally owned small business with a desire to give back to our communities in impactful ways!
