Job Description & Requirements Specialty: Home Health Discipline: RN Start Date: 05/25/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Local Contract Position Overview The Home Health Clinical RN is responsible for delivering skilled nursing care to patients in their homes, with a focus on OASIS assessments, care plan development, and ongoing return visits. This role supports patient recovery and independence through comprehensive clinical care, education, and coordination across disciplines. Schedule & Work Details Coverage Area: Clearfield and Centre County, PA Schedule: Monday through Friday, 8:00 AM to 4:30 PM Weekly Hour Guarantee: 36 hours On Call/Weekend Requirement: Yes Mileage Reimbursement: Yes, reimbursed between visits from first patient to last patient Pay Rate: $51 to $53 per hour Key Responsibilities Complete comprehensive OASIS assessments in the home setting Develop, implement, and update individualized patient care plans Perform skilled nursing return visits based on physician orders and patient needs Administer IV therapies and perform venipuncture Provide wound care, including management of wound vac systems Deliver disease process education and self care instruction to patients and families Monitor patient progress and report changes in condition to the care team Accurately document all visits and care provided in accordance with regulatory and agency standards Collaborate with physicians, therapists, and interdisciplinary team members to ensure coordinated care Maintain compliance with home health, state, and CMS regulations Required Qualifications Active Registered Nurse license in the state of Pennsylvania Proven home health and HBHC experience Strong working knowledge of OASIS documentation Experience with IVs, wound vacs, wound care, and venipuncture Ability to provide effective patient disease education Valid driver's license and reliable transportation About PRIDE Health PRIDE Health is the minority-owned healthcare recruitment division of Pride Global—an integrated human capital solutions and advisory firm.
