Active Pennsylvania Physical Therapy license required to start the assignment. Home Health experience required, including OASIS assessments and development of plan of care. Experience supervising physical therapy assistants preferred. Electronic charting experience required. HCHB documentation experience preferred. Reliable vehicle required for patient visits. Mileage reimbursed between visits at employer rate. BLS certification may be required from AHA or ARC. Minimum education requirement: Associate degree or higher. Assignment Details Facility Type: Home Health. Shift Name: Days. Shift Details: Monday–Friday 08:00–16:00 (days). Weekly Hour Guarantee: 36 hours per week. Productivity Expectations: 26 points. Coverage Areas: Clearfield and Centre Counties; travel between patient homes required. Primary responsibilities: OASIS assessments, home-based physical therapy treatment, development of plan of care, supervision of PT assistants, electronic charting.
