Responsibilities include:Serves as the primary point of contact for referrals, appointment scheduling, and linkage to appropriate levels of care. Screens and enrolls patients in the Opioid-Based Outpatient Treatment (OBOT) Program. Leads the client intake process, including screening, education/review of program expectations, and completion of enrollment paperwork. Provides complex care coordination across multiple life domains and disciplines, including communication with community partners, referrals to outside agencies, and management of other care needs. Coordinates care with our regular Medication for Opioid Use Disorder (MOUD) services to ensure seamless patient support. Uses motivational interviewing and shared decision-making to address barriers and promote full participation in treatment. Attends warm handoffs when patients in the clinic request contact with the OBOT program. Acts as the primary source for supporting the OBOT team in patient tracking. Ensures effective collaboration and communication between members of the healthcare team to support care coordination activities and program goals. Consults with medical and behavioral health providers and other team members to resolve client care issues. Highschool Diploma or equivalent required Professional Degree in Human Services preferred CADC certification or similar credential/experience in substance use treatmentCertified Medial Assistant required Excellent listening and communication skills Bilingual preferred Experience with EHRs At Blue Ridge Health our mission is to improve Health, inspire Hope, and advance Healing through access to Compassionate, Affordable, and Quality Care.
