This role requires a dynamic leader who is passionate about improving health outcomes for children, promoting family engagement, and ensuring the delivery of high-quality, coordinated careProgram Development and ManagementDevelop, implement, and manage programs and services for children and families within the Health Home modelEnsure that services are culturally competent, family-centered, and aligned with best practices and regulatory requirementsMonitor program performance and outcomes, making data-driven decisions to enhance service deliveryLead continuous quality improvement initiatives to optimize care coordination and patient outcomesLeadership and SupervisionSupervise and support a multidisciplinary team of professionals, including case managers, social workers, and support staffFoster a collaborative and inclusive team environment that encourages professional growth and developmentConduct regular team meetings, performance evaluations, and provide ongoing training and supportMentor staff in evidence-based practices and Health Home service delivery standardsCollaboration and Community EngagementBuild and maintain partnerships with community organizations, schools, healthcare providers, and other stakeholders to enhance service delivery and resource availabilityAdvocate for children's health and wellness initiatives within the community and at the policy levelRepresent the organization at community meetings, conferences, and other professional eventsDevelop strategic alliances to expand access to pediatric specialty services and community resourcesQuality Assurance and ComplianceEnsure compliance with all relevant regulations, policies, and standards pertaining to children's health services, including New York State Health Home requirementsImplement quality improvement initiatives to enhance service delivery and client satisfactionConduct regular assessments and audits to ensure adherence to program protocols and standardsMaintain accurate documentation and reporting systems for regulatory and accreditation purposesMaster's degree in social work, Public Health, Psychology, or related fieldCurrent New York State licensure (LCSW, LMHC, or equivalent) preferredMinimum of 5 years of experience in children's services, healthcare, or social servicesMinimum of 3 years in a supervisory or leadership roleStrong knowledge of the Health Home model, child development, and best practices in pediatric healthcareExperience working with diverse, underserved populationsProficient in data analysis and program evaluation methodologiesCore CompetenciesExcellent communication, interpersonal, and organizational skillsAbility to work collaboratively with diverse populations and stakeholdersStrong analytical and problem-solving abilitiesLeadership and team management skillsCultural competency and sensitivityKnowledge of trauma-informed care principlesEssen Health care is proud to be an equal opportunity employer, and we seek candidates who desire to work in and serve an ethnically diverse population.
