The RN Clinical/Case Manager will utilize the nursing process to provide skilled care to clients in their homes and is responsible for completing and documenting client visits as per RiverView Home Care, CMS requirements, and The Joint Commission standards. If not held currently, training will be provided and certification must be obtained within 4 months from the date of hire.Preferred:Two years+ experience in a clinical or home health setting.Experience with EPIC EMR.
