Physical Therapist- Monroe County

Salary Range: $70,000.00 - $85,000.00 Annually Role and Responsibilities A qualified professional, who directs, supervises, evaluates and provides physical therapy services to patients in the home , following physicianu2019s orders. Essential Functions + Evaluate patient using diagnostic muscle, nerve, joint , and functional ability tests and measures. + Prepare an assessment of the patientu2019s condition at the time of the initial evaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment. + Recommend modification of plan of care to physician, as needed. + Document clinical notes on the date of the visit into medical record and keep all records and summaries up to date . + Treat patients to address impairments and functional limitations , with a goal of reach in g maximum functional independence. + Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic , and durable medical equipment (canes, walkers, wheelchairs, etc.) . + Include and educate the pat i e n t and caregiver ( s ) regarding treatment plan and goals. + Attend physical therapy staff meetings and in-service presentations. + Participate in case conferences. + Participate in the development and periodic revision of the physicianu2019s Plan of Treatment. + Assure appropriate safety practices and maintenance of equipment. + Other duties as assigned. Education Requirements + Graduate of an accredited Physical Therapy program. Qualifications and Requirements + Current, valid Physical Therapy l icense issued by N YS Department of Education . + One year of practical experience as a licensed physical therapist. Work Environment The Physical Therapist is primarily in a non-office setting and may be exposed to outdoor conditions. The working conditions are classified as medium work: + Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.
