Position Status: Full time - Day shift (must be available 1 weekend / month)Position Location: Sentara Home Care Services - Gloucester, VA (travel within a 40 mile radius)OverviewThe Licensed Practical Nurse (LPN) in a home health care setting is responsible for providing direct, one-on-one care to patients in their homes. This role involves monitoring patient health, administering treatments, managing medications, and offering essential support to patients with various medical needs. Collaboration with a team of healthcare professionals, including Registered Nurses (RNs) and physicians, is required to deliver high-quality, compassionate care that improves patient well-being.EducationTrade School Graduate (Required)Certification/LicensureLPN License (Required) - Active in VirginiaBLS (Required within 90 days)ExperienceServe as patient advocate in the prevention of illness or diseaseProvide care and treatment for patients and clinical support and teaching for patient and family membersCustomer service, testing, medication administration and the implementation of nursing interventions and care plansSupport a holistic approach to patient care lpn, talroo-lpn, licensed practical nurse, gloucester, fulltime, indeed, home care, home healthWe provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay rate for Full Time employment is:$22.39 - $37.32. Additional compensation may be available for this role such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
