SupervisesLCSW, LMFT, ASW, AMFT, APCCEssential Job Functions Responsible for hiring, firing, promoting, training, evaluating and/or directing the work of designated staffProvide oversight of therapeutic services to ensure appropriate, timely,and quality patient/client careProvide clinical supervision to Associate Social Workers and Associate Marriage and Therapy Therapists and assist in assignment of clinical supervision duties to other existing LCSW’s or LMFT’s according to established legal and ethical codes of the California Board of Behavioral Sciences and VCC Policies and Procedures.Ensure patients have access to services within established timeframesConduct interviews with patients and families to gather information, identify adherence risk, and collect relevant health information from patient, assess patient’s social service needs, determine the appropriate need, and provide referrals for community resourcesCoordinate training for trainees/interns as neededProvide direct therapeutic services; perform crisis intervention services as neededReview and sign progress notes, treatment plans and other documents in client records on a weekly basis, ensuring compliance with governing agencies; review and sign progress notes as required for Clinical Supervision of Associate Staff (ASW/AMFT)Participate clinical staffing meetings to assist in coordination of care between therapists and medical cliniciansParticipate in QA activities and auditsEnhance professional growth and development of staff through participation in educational programs, current literature review, in-service meetings and workshopsAdhere to the established legal and ethical codes of the California Board of Behavioral Sciences and Policy and Procedure Guidelines of the department and agency as well as any contractual requirements; ensure department compliance with policies and standardsCollaborate with clinicians to promote and report adherence to medication and treatment plan by patientsEducate and coach patients on self-management of condition-related symptomsIdentify barriers to adherence and coach patients on solutionsLink patients to support systems and resources related to medication assistance, self-management, condition management, and psychosocial supportsAssist with the coordination of primary care or specialized treatment including the development of specialized resources, referrals and transportation to treatment clinicians and specialized services for the purpose of ensuring each patient is provided with specialized services to assist in his/her treatment planWork collaboratively to develop programs that improve health and health behaviors and disease management Collect data to monitor patient outcomes and support CQICoordinate with caseworkers, community, support services, and appropriate referrals; work with other agencies to obtain information and coordinate servicesAttend, participate in, and facilitate a variety of internal and external meetingsSupport the vision, mission and goals, and demonstrate a commitment to the values, of the organizationPerform other duties as directedMinimum Qualifications Bachelor’s degree, preferably with a major in Social Work, Psychology, Sociology or related behavioral scienceMinimum two years’ experience in a private or governmental agency providing direct behavioral health case management related services to patients, which should include identifying patient social service needs, using community resources, and patient advocacyMinimum five years’ experience in Behavioral Health managementValid California driver’s license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes;Preferred QualificationsBilingual English/SpanishLCSWPreferred experience of school-based servicesBoard Certified Supervisor (oversight of associates and interns)Required Skills/Knowledge/Abilities Familiarity with California Laws and regulations for mental health servicesNon-violent crisis intervention techniquesFamiliar with evidence-based practices and philosophyExperience with data collection and record-keeping techniquesExcellent verbal and written skills necessary for communication with patients/clients, clinicians and other staffAbility to interface with all levels of personnel in a professional manner, including people of all social, cultural and ethnic backgrounds and within the constraints of government funded programs Ability and willingness to meet the organization’s attendance and dress code policiesExperience/familiarity with computers, business e-mail, communication systems and internet search capabilities and proficient in Microsoft Office products, specifically Word and ExcelPay Range$125,000 - $135,000
