Now Hiring: Behavioral Health Navigator (BHN) - Make a Real ImpactSupporting Clinics in OregonAre you passionate about helping patients navigate the complexities of the healthcare system? At the outset, the BHN may provide support across two of our newest clinics. As a Behavioral Health Navigator (BHN), you will be an essential advocate for patients, helping them access critical services while working alongside an outstanding healthcare team.What You’ll DoServe as a vital link between patients and behavioral health, substance use disorder (SUD), and social services.Maintain a comprehensive database of mental health (MH) and SUD providers, ensuring up-to-date knowledge of available resources.Provide same-day support to primary care patients and respond promptly to referrals.Be accessible through multiple channels—phone, portal, and in-person—to assist patients and healthcare teams.Help patients navigate challenges related to social determinants of health (SDOH), including access to housing, legal services, and community resources.Collaborate with care teams, primary care providers, and local organizations to improve patient outcomes.What You BringMinimum 2 years of college education required; BA/BS preferred.Experience in a medical setting is highly desirable.Strong interpersonal skills with the ability to build trust and relationships with diverse patient populations.Excellent organizational skills and attention to detail, particularly when managing confidential patient information.Familiarity with Microsoft Office and electronic health records (EHRs).Why Join Us?Mission-driven work—help patients access life-changing care.Collaborative and supportive environment with a dedicated team.Opportunity for professional growth in a dynamic healthcare setting.Ready to make a difference?
