Job Description The IOM Specialist II requires a minimum of 1-5 yearsu2019 experience in Intraoperative Monitoring (IOM). As a key member of the surgical team, the IOM Specialist II works closely and collaborates with nurses, physicians, anesthesiologists and other medical staff caring for the patient. The IOM Specialist II conducts detailed neurophysiological studies of the central and peripheral nervous system during spine and peripheral nerve surgeries under direct or remote supervision. The techniques with which the IOM Specialist II must have expertise include the elicitation of somatosensory and motor-evoked potentials, and spontaneous and triggered EMG for surgical cases that place the nervous system at risk including spine surgeries (e.g. lumbar laminectomy, anterior cervical discectomy, posterior lumbar inter-body fusion) and peripheral nerve surgeries. IOM Specialist II will learn the techniques necessary for monitoring of neural function during cerebrovascular procedures (e.g. carotid endarterectomies, aneurysm clipping); microvascular decompression procedures; CPA tumor removal; surgery of jugular foramen and lower clivus; spine and spinal cord procedures (e.g., spinal deformities correction, extra and intradural tumors removal). These techniques include the elicitation of auditory evoked potentials, monitoring of cranial nerves, cortical mapping, and spinal cord mapping. Understands the important details of each case in order to select appropriate monitoring techniques. 2. Participates in the planning of intraoperative monitoring modalities appropriate to the surgical procedure. 3. Performs all pre-operative patient verifications, communications with surgeons and anesthesiologists, and documenting. 4. Selects montage(s) appropriate for surgical procedure being performed. 5. Prepares patients for IOM and acquiring signals. 6. Engages in timely discussions with the surgeons and anesthesiologists regarding all issues related to IOM. 7. Performs basic and moderately complex IOM as needed (e.g., spinal and peripheral nerve surgeries). 8. Sets up and confirming proper operation of all equipment including: a.Testing of equipment and checking integrity of electrodes b.Arranging head boxes, cables, and electrodes for minimization of artifacts and electrical hazards preventing electrodes from being dislodged, dried out, or contaminated with fluids c.Ensuring that all stimulators are delivering expected stimuli to the correct site. d.Assist technologists in troubleshooting electrical interference and software problems. e.Maintain equipment and perform simple repairs as needed. 9. Documents procedure, modalities used and tests performed, surgical maneuvers and events, levels of inhaled anesthetics, dosage of intravenous anesthetics, use of muscle relaxants, blood pressure, body temperature, and other physiologic parameters as appropriate. 10. Documents any and all communications or warnings relevant to patient care including corrective action taken. 11. When monitoring cases, follows standard Mount Sinai IOM program policies and protocols, including precautions for infection control per Mount Sinai policy and procedures, and standard Mount Sinai precautions for general safety and hazardous material management. 12. Completes post-operative reports and billing forms. 15. Performs other related duties.
