This full-time position supports our Radiation Oncology Clinic, playing a key role in helping patients navigate their care with empathy and efficiency.Job OverviewEnhance the patient experience by providing a seamless navigation process from abnormal finding/diagnosis through the cancer continuum. Collaborate with physicians and members of the interdisciplinary team to coordinate and consistently manage patient care by serving as a point of contact for patients and families. Systematically and continually perform the functions of assessment, planning implementing and evaluating care according to the nursing process and the Oncology Nursing Society Standards of PracticeEssential FunctionsCoordinates patient care through the cancer continuum by assisting with timely access to care, coordinating/facilitating appointments and services, and encouraging adherence to treatment plans, protocols and follow up careAssesses patients current and future needs, provides patient access to resources(internal and external) and makes appropriate referrals as neededProvides education and information to the patient and family related to the cancer healthcare system, multidisciplinary team member roles, plan of care and available resourcesEstablishes and maintains close collaboration with physicians and multidisciplinary team members to ensure seamless patient care and care coordinationTracks navigation program metrics, and quality indicators, documents patient interactions and progression and communicates navigation activities to physicians, cancer care team and departmentSupports patients and families during the transition from active treatment to survivorship or assists with coordination of end of life careContributes to the development, implementation and evaluation of the nurse navigator roleBuilds awareness of the nurse navigator role by participating in marketing and community outreach effortsObtains or develops patient and family educational materials and resourcesDemonstrates knowledge, skill and coordination to provide nursing care and guidance to the cancer patient from screening to survivorshipDemonstrates effective communication with patients, families, peers, members of the multidisciplinary healthcare team and community organizations and resourcesMaintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards.Maintains compliance with all Orlando Health policies and procedures.Actively participates in multidisciplinary planning conferencesParticipates in community health promotion and awareness programs (health fairs, screenings, symposiums)Attends staff development in-services, department meetingsEstablishes and maintains professional role boundaries with patients, families and multidisciplinary care teamServes as advocate for patient and familyEducation/TrainingMSN requiredAssumes responsibility for professional development and continuing educationMeets all mandatory and developmental requirements for Orlando Health and unit/departmentLicensure/CertificationMaintains license as an RN in the state of FloridaONS certification required.Maintains current BLS/healthcare provider certificationExperienceFive (5) years clinical experience. Including two (2) years as an oncology patient navigator.
