The Care Team Specialist (CTS)participates as a member of a care team and supports high qualitypatient care including engaging patients in maintaining their physicaland mental well-being. The Care Team Specialist performs technical,clinical, and administrative tasks which contribute to the smoothworkflow and quality patient care experience in the clinic.Responsibilities include, but are not limited to pre-visit preparation,monitoring patient flow, rooming patients, obtaining patient historiesand vital signs, managing certain test results following protocols,supporting Epic in-basket workflow, returning phone calls, and assistingwith medical examinations and procedures. The Care Team Specialistassists and collaborates with the Registered Nurses and MedicalProviders in the management and coordination of patient care. The CTSstaff will participate in the review of team/provider in-baskets and MyChart messages for the assigned care team. Practitioner and RegisteredNurse staff may delegate assignments to the Care Team Specialist. CTSstaff at GHC-SCW will have successfully completed medical assistanttraining, licensed practical nurse training, Paramedic training, or EMTtraining from an accredited program. Job Specific Minimum QualificationsMust meet the requirements for a Medical Assistant (MA), a LicensedPractical Nurse (LPN), or an EMT as described below: Medical Assistantsmust have successfully completed an MA training program from anaccredited school or program and must be eligible for and receivenational certification within six (6) months of employment or must havethree years of MA experience and have current national certification.Medical Assistants must obtain national certification from one of thefollowing certifying organizations and must maintain certification bymeeting CME...
