Job Description Job Title: Peer Bridger - Peer Bridger Program - Mount Sinai West - Full Time - Day The purpose of the Peer Bridger is to provide essential bridging between the Emergency services delivered during the crisis, a more lasting services over the longer term. Responsibilities for both these positions include participating in collaboration and coordination meetings, delivering recovery strategies, responding to patient emergencies as needed, as well as providing a direct link to service providers Qualifications Education Requirements + Hold current/active New York Peer Certification. + New York Certified Peer Specialist, Provisional (NYCPS-P). + New York Certified Peer Specialist (NYCPS). + New York Certified Recovery Peer Advocate (CRPA). + Credentialed Family Peer Advocate (FPA-C). + Credentialed Youth Peer Advocate. Experience Requirements + Demonstrate ability to navigate community resources. + Ability to model, facilitate, and convey the message that healing is possible and probable. + Exhibit oral, reading, writing, and computer skills with basic knowledge in applications such as Microsoft Office: Teams, Word, Power Point, Excel, Outlook. + Demonstrate excellent interpersonal and communication skills with ability to work independently and as part of a team + Ability to maintain confidential information as related to position, always maintaining transparency with any limits that are identified. + Healthcare and/or Behavioral Healthcare experience preferred. + Formal training as Peer or equivalent work experience. Compensation Statement The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $25.32 - $37.97 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits. Non-Bargaining Unit, RAR - Peer Bridger Program - WST, Mount Sinai West Responsibilities + Offers hope through connection and engagement on what is possible. + Supports individuals in connecting or reconnecting to their community and relationships. + Connects to and support individuals in various stages of navigation from CPEP to community based on identified needs and requests for support. + Provides validation and support through person centered approaches and engagement. + Advocates when requested by the individual. + Assists in the development of comfort around self-advocacy. + Demonstrates personal self-help and wellness skills. + Shares known resources and knowledge on system navigation. + Serves as a mentor when appropriate on wellness and recovery. + Assists in the exploration of personal goals that lead to life satisfaction. + Engages in shared decision making with individuals to support voice and choice in all treatment decisions. + Utilizes evidence-based peer interventions such as Action Planning for Prevention and Recovery, Shared Decision Making, Psychiatric Advanced Directives, Building Recovery Capital, and the Eight Dimensions of Wellness. + Conducts informational groups for CPEP staff about the benefits of working with Peer Bridger. + Works collaboratively with the team to educate all involved on recovery, self-determination, shared decision making, dignity of risk, and peer values. This position will work out of Mount Sinai Rivington Behavioral Health Center, located at 45 Rivington Street New York, NY 10002.
