This position will give you the opportunity to provide exceptional care and show compassion to patients residing on the west side of Cleveland and surrounding areas. In this role, you will provide assessments, complex nursing treatments, and education to help avoid readmissions to the acute care setting. After seeing patients, Home Health RNs work remotely to complete documentation, make calls, attend meetings, coordinate patient care and arrange visit schedules. Cleveland Clinic Home Care provides a generous in-depth orientation that is tailored to each individual and their experience level. We provide our nurses with many professional development resources, including furthering education and online/in-person courses to enhance your skills and eligibility for student loan repayment.A caregiver in this position works days from 9:00 a.m. - 5:00 p.m. Caregivers in this role are required to work one weekend every four weeks, two holidays per year and one on-call shift per week. A caregiver who excels in this role will:Provide direct nursing care. Perform accurate and complete admission assessments and reassessments within a specified time frame.Assess patients for safety risks, abuse and neglectServe as primary coordinator of all disciplines for well-coordinated patient care Assess, document, prioritize and manage patients’ painMinimum qualifications for the ideal future caregiver include:Graduate from an accredited school of professional nursingCurrent state licensure as a Registered Nurse (RN)One year of clinical experience as an RN in an acute med/surg settingValid Ohio driver’s license and proof of automobile insurance coverage with $100,000/$300,000 coverage Preferred qualifications for the ideal future caregiver include:Bachelor of science in nursing (BSN). Must be able to work in settings with infectious diseases.Must present a professional appearance at all times and attend staff and agency meetings, as requested.Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects.
