JOB REQUIREMENTS: Population Health Outreach Specialist Job LocationsUS-WI-Madison ID 2025-2262 Category Clinical Services Type RegularFull-Time Overview Come Find Your Spark at Quartz The Population HealthOutreach Specialist will play a critical role in empowering communities,connecting individuals to resources, and improving health outcomes byconducting phone calls and text messaging outreach services to informmembers of actions related to recommended tests or screenings or tosecure clinical data on behalf of care teams that meet medical,behavioral health, and social needs of Quartz members. This positionwill build individual and community capacity by increasing healthknowledge and self-sufficiency through a range of activities such asoutreach, community education, informal counselling, social support, andadvocacy. Benefits: Ability to impact a large and diverse memberpopulation including those in rural and underserved/hard to reach areas.Data driven outreach identifying those most in need of care with aproactive approach to keep the population healthy Flexible work/lifebalance with options to work in-person, hybrid, or remotely Startingsalary is based upon skills and experience: $50,900 - $63,700 withrobust benefits package Responsibilities Conducts phone call and textmessaging outreach to members in priority populations to improve memberexperience and clinical, quality outcomes. Organizes information frommembers' health history and internal data sources to develop anoutreach approach to address member needs and ensure implementedinterventions are collaborated with members and their providers.Collaborates with Quartz clinical services, local clinical team, socialservice providers, and community stakeholders to support the member'scare plan. Provides referrals for community resources and socialservices. Reviews, monitors, and updates quality program plans,materials, and reports. Documents member information and contactscompleted regarding the members' care and services provided. Activelymonitors the members' ability and willingness to engage in treatmentregimes, and problem solve with staff, members, and providers, offeralternatives or solutions to meet their needs. QualificationsBachelor's degree with 2 years of experience in healthcare or socialwork OR associate degree with 5 years of experience in healthcare orsocial work OR High school equivalency with 8 years of experience inhealthcare or social work Able to work independently in Microsoft OfficeApplications Able to learn documentation systems such as Epic Knowledgeof Medicare and Medicaid benefits Effectively communicate thoughts,ideas, and information both orally and in writing Able to demonstrateflexibility, set priorities with daily demands and...
