Better Health.Overview:The APRN is a clinical role that will provide exceptional medical care to patients by conducting physical exams, ordering and interpreting diagnostic tests, and developing patient treatment plans under the supervision of a physician. This role is a safety-sensitive position as it involves direct patient care and administering medication. The incumbent will report to the Regional Manager (or similar role) and will work closely with other healthcare professionals to provide quality care to patients collaborating with other clinical teams to support team function.Responsibilities:Practice medicine under the supervision of a physician using sound medical judgment and adhering to all ethical considerations of the practice of medicine; knowing limitations and seeking consult when advisable; keeping abreast of current technologies and industry standardsUnder the supervision of a physician, provide care for patients with acute and chronic illness encountered in the older adult patientUnder the supervision of a physician, accountable for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will stay involved in, and be responsible for, the detailed care of the patientEngage with hospitalists whenever a patient is in the hospitalMeet all clinical obligations of the medical practice: Accrediting Agency compliance, CLIA compliance, drug prescription compliance, HIPAA, State and Federal regulations, Safety Regulations, etcAssess, diagnose, treat, manage, educate, promote health, and provide care coordination and documentation for patients with acute and complex chronic health needs using evidence-based medicine and practice guidelinesFollow all internal policies and proceduresAccountable for the completion of all operational tasks according to departmental guidelines to ensure appropriate clinical processing/documentationMaintain complete and detailed records, electronically in the EMR, of patients’ health care plans and prognosis, completed within 48 hours of each patient’s visitProvide patients with information needed to promote health, reduce risk factors, or prevent/manage disease or disabilityAnalyze and interpret patients’ histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnosisInterpret data and diagnostic results for an appropriate course of action including but not limited to blood gases, radiology, EKG, etc. in collaboration with supervising physicianExhibit behavior that emphasizes compassion, quality, loyalty, and conscientiousnessAbility to establish and maintain effective working relationships with patients, their families, and team membersDocument continuing education per licensure and certification requirementsEnsure that patients are seen every other visit by the PhysicianMaintain patient confidentiality in accordance to policy and procedure and HIPAA requirementsEnact proper chain of command for patient complaintsAdditional duties as assignedPosition Requirements/ Skills:Graduate of an accredited school of nursingCertified as a nurse practitioner with current state licensureAt least 1 year of previous professional nursing experience with 1-3 years experience as a practicing as a primary care nurse practitionerPrior experience working in an electronic medical records systemProvide 5 Star service in accordance with the Better Health Group's missionSkilled in traditional nursing disciplines of patient assessment and managementACLS/BLS certification required as mandated by stateMust demonstrate knowledge of the rationale of appropriate patient careDemonstrate ability to handle emergency situations calmly and effectivelyAbility to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulationsAbility to effectively present information and respond to questions from a various group of peopleAbility to define problems, collect data, establish facts, and draw valid conclusionsExcellent written and verbal communication, teaching ability, and good telephone skillsWorking knowledge with Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint) for real-time collaborationPhysical Requirements:Full range of body motion, including handling and transferring patients, manual and finger dexterity;
