No County Richmond Street Address South Beach Psychiatric Center 777 Seaview Avenue City Staten Island State NY Zip Code 10305 Duties Description South Beach Psychiatric Center is recruiting an Intensive Case Manager to provide case management and care coordination services to individuals with mental illness in the community. Candidates from outside or within State Service can apply.As an Intensive Case Manager, your duties will include: u2022 Assessing individuals, developing individual care plans, and determining necessary resourcesu2022 Advocating for and assisting individuals in accessing and obtaining servicesu2022 Contacting service providers to determine appropriate placements and referralsu2022 Monitoring individual rehabilitation goals and progressu2022 Providing crisis intervention services Minimum Qualifications Candidates from outside or within State Service can be considered for hire under the Hiring for Emergency Limited Placement u2013 Statewide (NY HELPS) program as an Intensive Case Manager if they have:u2022 Bacheloru2019s degree in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, or speech and hearing; ANDu2022 4 years of experience providing direct services to individuals, 50% of whom have been diagnosed with mental illness, developmental disabilities, and/or substance use disorder or 4 years of experience linking individuals, 50% of whom have been diagnosed with mental illness, developmental disabilities, and/or substance use disorder, to a broad range of services essential to living successfully in a community. 1 of the 4 years must have included working with individuals, 50% of whom have a primary diagnosis of mental illness.Substitution: Master's degree in a related field can substitute for 1 year of specialized experience.ORCandidates from outside or within State Service can be considered for a competitive class appointment if they are reachable on the current Intensive Case Manager eligible list.ORCandidates within State Service can be considered for a competitive class appointment if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.
