This role offers a unique opportunity to provide essential therapy services in the comfort of patients' homes, contributing positively to their overall health and well-being.u003c/pu003eu003cpu003eu003cstrongu003eKey Responsibilities:u003c/strongu003eu003c/pu003eu003culu003eu003cliu003eImplementing physical therapy programs as directed by a licensed Physical Therapist to help patients achieve their mobility and functional goals.u003c/liu003eu003cliu003eConducting home visits to provide therapeutic treatments and exercises to improve patients' physical function and independence.u003c/liu003eu003cliu003eMonitoring patient progress and making adjustments to treatment plans as needed, in consultation with a supervising Physical Therapist.u003c/liu003eu003cliu003eEducating patients and their families on exercises and activities that can aid in recovery and improve quality of life.u003c/liu003eu003cliu003eDocumenting patient care activities and progress accurately and in a timely manner.u003c/liu003eu003c/ulu003eu003cpu003eu003cstrongu003eQualifications:u003c/strongu003eu003c/pu003eu003culu003eu003cliu003eAssociate Degree from an accredited Physical Therapist Assistant program.u003c/liu003eu003cliu003eCurrent PTA license in the state of Texas.u003c/liu003eu003cliu003ePrevious experience in a home health setting is preferred.u003c/liu003eu003cliu003eStrong interpersonal and communication skills to effectively interact with patients, families, and healthcare team members.u003c/liu003eu003cliu003eAbility to work independently, manage time efficiently, and adapt to changing situations in a home care environment.u003c/liu003eu003cliu003eValid driver’s license and reliable transportation for home visits.u003c/liu003eu003c/ulu003eu003cpu003eu003cstrongu003eWorking Conditions:u003c/strongu003eu003c/pu003eu003culu003eu003cliu003eThis is a full-time position with a flexible schedule to accommodate patient needs.u003c/liu003eu003cliu003eMust be physically able to perform the duties of the position, including lifting, transferring, and providing manual therapy to patients.u003c/liu003eu003c/ulu003eu003cpu003eJoin our team and make a meaningful impact on the lives of individuals in the Brenham community by providing exceptional home health physical therapy services.
