Badgercare Plus Coordinator

JOB REQUIREMENTS: Group Health Cooperative of South Central Wisconsin.BETTER TOGETHER 1.00 FTE / 40 hours per week. The BadgerCare PlusCoordinator serves as the designated HMO advocate and works with bothmembers, Group Health Cooperative of South Central Wisconsin (GHC-SCW)staff and providers to assist members in navigating the health caresystem through outreach and connection to community resources. Theyassist other GHC-SCW departments as to the benefits and/or enrollmentstatus of GHC-SCW members when issues of BadgerCare Plus coverage oreligibility arise. The BadgerCare Plus Coordinator will recommend andassist in the design of programs to improve various access issues, suchas timely prenatal care, completion of well child exams and otherclinical measures such as tobacco cessation, diabetes and asthma andaddressing Social Determinants of Health (SDoH). They are responsiblefor helping to remove barriers to healthcare delivery for GHC-SCWBadgerCare Plus members through outreach. They will work closely withAccess Community Health Centers, UW Family Medicine Clinics, and schooldistricts within the BadgerCare Plus service area to coordinate
