The CRN is responsible for providing clinical expertise to defined areas of the Medical Center.Communicates and collaborates with all members of the interdisciplinary team to provide coordinated care to the patient during unit huddles and multidisciplinary roundsMaintains a cooperative relationship among health care teams by communicating information, responding to requests, building rapport and participating in team problem solvingCollaborates and gives clinical input for appropriate bed placementPrioritizes patient and team member needs based on clinical situationProactively rounds on all clinical units and patients as appropriateProvides proactive management of patients at risk for physical and verbal escalation achieved through surveillance and interaction with patients and team members. Includes provider report, house rounding to identify patients of concern. Assesses and provides recommendations. Assists with communication and interventions of the care team. Assists with clinical troubleshooting. Assists with high risk interventions.Assists and role models during escalation emergencies and restraint situations. Assesses and stabilizes patients who are escalating in collaboration with the care team.Assumes a collaborative role, directing and assisting team members in setting priorities for patient careUtilizes a broad knowledge base to effectively triage multiple time sensitive requests, In the event of multiple calls for assistance.Partners with the interdisciplinary care team.Provides mentorship for the interdisciplinary team.Role models critical thinking skills, Nursing Interventions, Communication and collaboration skills, Use of HRO tools. Serves as a role model to establish positive relationships with team members and patients. Maintains a presence post call, follows up with the care team on patient status, facilitates debriefing. Provides post emergency debriefing.Participates in quality improvement, chart reviews, and productivity reporting, data collection includes documentation to capture data related to escalating situations.Offers support to the unit RNs with challenging assignments, procedures, and/or active changes in a patient's condition.Serves as a role model and leader to other team members in the organization. Maintains professional and technical knowledge by attending internal educational workshops, reviewing professional publications and participating in committee/council activities.Educates and certifies team members in de-escalation techniques and behavioral concepts.Other duties and/or projects as assigned.Adheres to HMH Organizational competencies and standards of behavior.Education, Knowledge, Skills and Abilities Required:Graduate of an accredited School of NursingCurrent registered nurse license in the State of New JerseyMinimum of 3 years of nursing experienceExhibits self-direction and use of a broad knowledge base, critical thinking, delegation, problem solving skills and protocol developmentExceptional verbal and non-verbal, written and listening skills. Ability to adapt communication style to suit different audiences and function in a collaborative roleOrganizational skills with the ability to handle multiple complex situations simultaneouslyProficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platformsEducation, Knowledge, Skills and Abilities Preferred:Behavioral Health experience.Licenses and Certifications Required:NJ State Professional Registered Nurse License.AHA Basic Health Care Life Support HCP Certification.Licenses and Certifications Preferred:Crisis Prevention Intervention Instructor Certification.Minimum rate of $106,329.60 AnnuallyHMH is committed to pay equity and transparency for our team members.
