At Health Connect America, we are dedicated to making meaningful connections every day through creating quality, affordable opportunities for individuals and families to achieve their greatest potential in a safe, positive living environment.Come make a difference and grow with us!Our BrandsThe Family Support Specialist provides best practice, quality clinical services, ensures all documentation is completed per Payer’s, state licensing agent’s, COA, & HCA’s standards and established guidelines and assist with marketing for new referrals.Communicate with customers, colleagues, and other individuals to answer questions, disseminate, or explain information, and address complaints/concerns.Develop treatment plans with the input of the treatment team and the Clinical SupervisorProvide individual, group, and family therapeutic interventions and skill building as outlined in the Treatment PlanImplement Health Connect America approved therapeutic interventions as delineated on the treatment plan with clients and their familiesComplete all client documentation within established time framesAttend and actively participate in weekly Treatment Team meetingsLink the client and family with specific services and resources as identified in the treatment planServe as a liaison with other professionals and agencies as related to the clientActively recruit new referrals to ensure that your caseload stays fullProvide 24/7 on-call support for clients as per program requirementsEnsure compliance with all state regulatory bodies & COA standards.Participate in and complete all assigned training and professional development activities, including E Learning courses.A Bachelor's degree in a human services discipline such as Social Work, Psychology, Sociology, Counseling, or Criminal Justice, along with 1 year of experience working with children (including internships or volunteer experiences) is preferred, OR a BA/BS degree with 3 years of relevant experience.
