Our goal is to improve the lives of the people we serve and help keep them at home, by providing best-in-class disability competent primary care. u2022 Reporting to a member of Care Delivery Leadership, the Primary Care Nursing Manager is the nursing leader of the practice. Responsible for all nursing and nursing support functions within the practice, the Primary Care Nursing Manager will collaborate closely with other members of the management team. This role is full-time on site in our Boston clinic and will assume the primary responsibility for clinical operations. Supervision Exercised: u2022 Assumes responsibility for all nursing (RN, LPN) and MA roles in the practice. 10 u2013 15 direct reports across multiple sites. Will require periodic travel to Springfield and Worcester locations, as needed, as well as supervision of fully remote employees. Essential Duties & Responsibilities: Clinical Leadership and Patient Care u2022 Oversee delivery of safe, high quality, evidence-based patient care u2022 Manage daily nursing operations and patient flow in clinic and for nurses deployed in the community. u2022 Manage staff on triage line, ensuring adherence to clinical protocols and performance goals. u2022 Monitor compliance with clinical protocols, infection control and regulatory standards. u2022 Serve as clinical resource and role model for nursing staff. u2022 Support patient-centered care, ensuring timely service and satisfaction. u2022 Serve as a liaison between nursing staff, providers, and leadership u2022 Resolve patient, family and staff concerns in a timely and professional manner. u2022 Recommend resource allocation to meet patient and clinic needs u2022 Oversee clinical supply management inventory and control costs without compromising quality of care Staff Management & Development u2022 Recruit, hire, onboard and retain quality nursing staff u2022 Create staff schedules to ensure adequate coverage. u2022 Exercise very strong organizational, time management, and problem-solving skills. u2022 Feel comfortable leading interdisciplinary primary care teams and using judgment to deploy available resources appropriately. u2022 Act as point person and facilitator in the delivery of primary care to a defined panel of complex patients u2022 Subject matter expert for clinical and care coordination needs. u2022 Evaluate and assess health care needs utilizing established triage protocols and documented workflows. u2022 Escalate issues to APC/physician, or other appropriate team members, based on triage protocol disposition, documented workflows, or nursing judgment. u2022 Maintain timely and accurate documentation. u2022 Oversite for coordination and scheduling of medical appointments u2022 Oversite of education to patients and caregivers regarding chronic disease management and follow-up, preventive health maintenance. u2022 Oversite of closure of quality gaps to achieve goal performance as defined by organizational targets u2022 Oversite of coordination of referrals, specialty visits, testing, social services, skilled services. u2022 Oversite of communication and follow up with skilled services like VNA on new/routine orders and chronic disease management. u2022 Work closely with other management team members in the delivery of primary care. u2022 Accountable for post-discharge follow-up and coordination of care. u2022 As a member of the management team, shared accountability for reductions in acute care utilization u2022 Attend team meetings and huddles to provide input on patient care, coordinate and complete action items. u2022 Utilize nursing judgement and/or provider orders to refer to internal and external auxiliary resources. u2022 Oversite of logistical planning for vaccination efforts or other clinical initiatives. u2022 Oversite of lab vendor relationships and follow-up as delegated. u2022 Complete other duties as assigned by the area leader. u2022 Have a commitment to observing, reporting, and enhancing patient and employee safety as part of an overall emphasis on continuous improvement, learning, and member experience in the organization. u2022 Onsite in our Boston clinic 4-5 days per week and serves as delegate Site Administrator for DPH licensure. u2022 Reporting and data management as needed for reports and areas of oversite. Working Conditions: u2022 This is a full-time clinic-based position. It is patient-facing. After on-boarding, there may be 1 day/week the role can work from home if adequate on-site coverage can be ensured. Laptop, Monitor, Headset, Phone, Microsoft Office, ShoreTel, Skype for Business, Guiding Care, EClinicalWorks, all programs that are related to documentation. Required Education (must have): u2022 Bacheloru2019s degree in nursing required Desired (nice to have): u2022 Masteru2019s degree in nursing Preferred Required Licensing (must have): u2022 An active and unrestricted MA RN license Desired Licensing (nice to have): u2022 CPQH or other quality certification Required Experience (must have): u2022 Meaningful clinical experience including: u2022 3+ yearsu2019 experience as a Registered Nurse in a high-touch clinical environment or home care, OR u2022 2+ years caring for patients/members with complex medical, behavioral health, and social needs u2022 Primary Care experience Desired Experience (nice to have): u2022 1+ years in a Nursing leadership role u2022 Experience managing DPH-licensed clinics u2022 Experience in value-based practice settings Required Knowledge, Skills & Abilities (must have): u2022 Must have the ability to handle stressful emergency situations calmly and effectively with patients, families, and health professionals. u2022 Must have the ability to de-escalate situations. u2022 Must have the ability to effectively work with patients with various cognitive, physical, and behavioral abilities u2022 Capacity to function independently within licensed scope of practice. u2022 Critical clinical thinking skills required. u2022 Excellent communication skills, i.e., listening, oral and written English skills while conversing. u2022 Willing to learn and utilize telehealth technologies (video, chat, etc.), when appropriate, for a variety of clinical care and care management services.
