JOB REQUIREMENTS: JOB PURPOSE AND REPORTING STRUCTURE: Under thedirection of the CCS Program Manager & Clinical Coordinator, the CareCoordinator, assists individuals diagnosed with a mental illness and/orsubstance use disorders to live independently in the community, inaccordance with agency policies and procedures. The Care Coordinatorwill assist individuals to utilize professional, community, and naturalsupports to address their needs both at home and in the community ontheir path to recovery. ESSENTIAL DUTIES AND RESPONSIBILITIES includethe following. Other duties may be assigned. Completes the MH/AODAFunctional Screen online training and 20 hours (or 40 depending on priorsocial service experience) of training provided by Milwaukee County CARSor in the community within 90-days of hire as outlined by DHS 36.MH/AODA Certification must be renewed every two years. Interviewsclients to complete the Mental Health/AODA functional screen and developan individual recovery plan, to include case planning, obtaining andreferrals for services, on- going monitoring, modeling, and servicecoordination. Assist clients developing client centered goals andservices such as: medical and mental health assistance, obtaining legalassistance and benefits, medication management, in finding employment,training and education, financial management and budgeting, ADLassistance, development of social support systems, AODA services andsupport, etc. Manage caseloads to provide supportive contacts and assistclients in navigating through the community. Assess clients needs andcomplete referrals and meet clients where they are in the community, toinclude hospitals within established guidelines. Provides services suchas care coordination as outlined by DHS 36 and Forward Health. Assistclients with developing life skills helpful for independent pletes discharge paperwork if client meets predetermined dischargecriteria. Transfer clients when services are no longer required or ifmore services are needed. Attend CCS Operation meetings for transfers ordischarge of clients. Coordinate/provide transportation for clients toand from appointments, to include medical and mental health, shopping,housing, etc. Attend and actively participate in departmental meetingsand treatment team meetings with other service agencies/plete regular billing/documentation for T-19 reimbursement ofservices provided to each client.
