No County Franklin Street Address 2445 State Route 30 City Tupper Lake State NY Zip Code 12986 Duties Description PRIMARY DUTIES AND RESPONSIBILITIES: u2022 Performs a wide variety of social services and support for assigned individuals. u2022 Independently provides written, verbal assessments, and service programs and provides written and oral input to treatment team meetings.u2022 Monitors individualu2019s programs to ensure continuity, consistency and progress as determined by the Interdisciplinary Treatment Team as related to the social work scope of needs.u2022 Responds to emergency situations as needed. u2022 Attends and participate in treatment team meetings. u2022 Completes admission and discharges of individuals which includes required paperwork and follow-up. u2022 Completes and submits, 30-day, quarterly and annual assessments in advance of treatment team meetings. u2022 Attends and provides input and assessment at individualu2019s treatment team meetings.u2022 Provide individual counseling sessions as needed.u2022 Ensures and provides documentation of services delivered, including interventions, reflecting needs, strengths, goals, level of participation and rights.u2022 Advocate for the needs of the individuals. u2022 Coordinates and arrange family visits. u2022 May accompany individuals on family visits, and must complete all home site assessments.u2022 Obtain, monitors, and follow-up on consents for RRC and ICC in a timely manner ensuring that they are current and complete. u2022 Functions as a liaison between individualu2019s families and the team.u2022 Obtains informed consent from individuals/families/legal guardians for any rights/restrictions or restriction intervention. u2022 May teach classes.u2022 Complete and upload MMAs and PEPs.u2022 Compile LRAC/SFAC packets and community referral packets as needed for individuals. u2022 Assist with self-advocacy group.u2022 Other duties as assigned. LinkedIn: Minimum Qualifications Promotion: one year of service as a Social Work Assistant 1.Open Competitive:A bachelor's degree in a human services field and two years of social work experience.Substitution:Master's degree in a human services field can substitute for one year of specialized experience. Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status.
