In this role, you will deliver high-quality nursing care to patients in the comfort of their own homes—performing assessments, providing complex treatments and offering education to help prevent hospital readmissions. Following patient visits, you’ll work remotely to complete documentation, coordinate care, schedule visits and collaborate with the interdisciplinary team. Cleveland Clinic Home Care offers a comprehensive, individualized orientation program and eligibility for student loan repayment, ensuring you have the support needed to succeed.A caregiver in this role will work days from 8:00am to 5:00pm.A caregiver who excels in this role will:Provide direct nursing care.Perform accurate and complete admission assessments and reassessments within a specified time frame.Assess patients for safety risks, abuse and neglect.Serve as a primary coordinator of all disciplines for well-coordinated patient care.Assess, document, prioritize and manage patients’ pain.Minimum qualifications for the ideal future caregiver include:Graduate from an accredited school of Professional NursingCurrent state licensure as a Registered Nurse (RN)Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red CrossValid Ohio Driver's (VDL) license upon hire and per policy 8-175 thereafterProof of $100,000/$300,000 automobile insurance coverage upon hire and per policy 8-175 thereafterOne year of current experience as an RNOne year of experience in an acute Medical/Surgical or Home Care settingPreferred qualifications for the ideal future caregiver include:Bachelor’s of Science in Nursing (BSN)Our caregivers continue to create the best outcomes for our patients across each of our facilities.
