JOB DESCRIPTION Job Summary Provides support for member health assessment activities. Collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum, including behavioral health and long-term care for members with high-need potential. Strives to ensure member progress toward desired outcomes and contributes to overarching strategy to provide quality and cost-effective member care. Essential Job Duties u2022 Utilizes collaborative process of assessment, planning, implementation and evaluation to engage, educate, and promote member decisions related to achieving and maintaining optimal health status. u2022 Engages nursing input/collaboration for member care planning. u2022 Supports members with moderate to complex clinical needs. u2022 Conducts standardized health risk appraisals (HRA) and applicable assessments, including verification of medical history and current health and wellness needs. u2022 Meets daily production standards in terms of member outreach. u2022 Utilizes web-based software system to document all assessments, activities and education provided. u2022 Develops problem list and individualized care plan using program protocols based on the members needs and preferences. u2022 Conducts telephonic education to promote self-management strategies for all applicable conditions. u2022 5-15% local travel may be required (based upon state/contractual requirements). Required Qualifications u2022 At least 2 years experience in health care, including at least 1 year experience in a care management, medical or behavioral health setting, or equivalent combination of relevant education and experience. u2022 Licensed Vocational Nurse (LVN), Licensed Practical Nurse (LPN), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Master Social Worker (LMSW). If licensed, license must be active and unrestricted in state of practice. Clinical licensure and/or certification required ONLY if required by state contract, regulation or state board licensing mandates. u2022 Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job related travel requirements, unless otherwise required by law. u2022 Demonstrated knowledge of community resources. u2022 Proactive and detail-oriented. u2022 Ability to work within a variety of settings and adjust style as needed - working with diverse populations, various personalities and personal situations. u2022 Ability to work independently, with minimal supervision and demonstrate self-motivation. u2022 Responsive in all forms of communication - remains calm in high-pressure situations. u2022 Ability to develop and maintain professional relationships with individuals. u2022 Excellent time-management and prioritization skills, ability to focus on multiple projects simultaneously and adapt to change. u2022 Excellent problem-solving, and critical-thinking skills. u2022 Strong verbal and written communication skills, including professional phone etiquette. u2022 Microsoft Office suite/applicable software program(s) proficiency, and ability to learn new programs. Preferred Qualifications u2022 Licensed Clinical Social Worker (LCSW), Advanced Practice Social Worker (APSW), Certified Case Manager (CCM), Certified Health Education Specialist (CHES), Licensed Professional Counselor (LPC), Licensed Professional Clinical Counselor (LPCC), Respiratory Therapist (RT) or Licensed Marriage and Family Therapist (LMFT). If licensed, license must be active and unrestricted in state of practice.
