Together, we keep getting better - advancing our mission to transform healthcare and serve as a leader ofpositive change.The Outreach Nurse Navigator, JTCC guides patients through the health care system by conducting community outreach and education and promoting cancer preventive screenings for our catchment area comprising the JTCC catchment area. The navigator works in partnership with the Outreach, Research and JTCC clinical teams to proactively identify and provide for patients' needs. This role plays a key role in establishing and maintaining JTCC's reputation in the community and trust with the patient. Often the first contact with the patient, this role is critical in ensuring that patients are informed of critical cancer screenings, are not lost to barriers or unable to receive services. A day in the life of an Outreach Nurse Navigator, JTCCatHackensack Meridian Healthmay include:Tracks screening and outreach activities, interventions and outcomes in a database. Facilitates relationships with community partners and service providers. 3. Provides and promotes cancer-related health education including clinical trials, identifies and works to reduce patient barriers, facilitates appointments, conducts eligibility screening and patient compliance for recommended preventive cancer screenings. Provides resources to help patients with social issues. Assists patients with making follow-up appointments and filling their prescriptions. Helps patients fill out applications for benefits. Analyzes health system data to support the program and identify patients who are at high-risk for poor outcomes. Explains and enrolls patients into the program, as a supplement to CHW initiated enrollments. Coordinates program activities including assistance with CHW recruitment, hiring and training. Analyzes data collected by the CHWs and generates reporting of program metrics to key stakeholders. Maintains and updates project plan in support of project operations. Provides administrative support for policy documentation processes. Works with other CHWs and staff to create a directory of community resources. Prepares reports and documents as needed or requested. Other duties and/or projects as assigned. Adheres to HMH Organization competencies and standards of behavior. BSN degree from an accredited college or universityNJ State Professional Registered Nurse LicenseExcellent organization skillsExcellent written and verbal communication skillsProficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platformsAbility to carry out written and oral instructionsAbility to exercise judgment in the application of professional servicesExperience with Data collection/management /analysisExperience service in low-income communities preferredMinimum rate of $111,924.80 AnnuallyHMH is committed to pay equity and transparency for our team members.
