Summary The Ukiah VA Clinic offers a wide range of services to veterans living in the Mendocino County community. The primary care clinics address preventative health care, health maintenance needs and primary care to eligible veterans. Specialty services include Dermatology, Nutrition, and Podiatry; in addition, veterans are able to be seen by Liver Service and Pulmonary Service Providers located at the San Francisco VA Medical Center through the Telehealth program. Responsibilities This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more The Ukiah RN Care Manager is a member of the Patient Aligned Care Team (PACT) Core Team (Patient, RN, LVN, MSA) and Expanded Team (family, care giver, internal and community based services) involved in providing care to the patient, developing and executing the patient centered care plan. This RN works in partnership with an assigned panel of patients and teamlet members for effective disease and chronic care management. The Ukiah RN Care Manager is responsible for collaborating with services at the VA, both external and internal, to facilitate care transition and improve patient care outcomes. Duties and responsibilities of the RN Care Manager include: Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. Provides patient care to veterans in keeping with Department of Nursing standards and with attention to physical, emotional, social, spiritual, cultural, and age-specific needs while preserving personal dignity and autonomy. Demonstrates safe expert knowledge and care relevant to respective nursing area and the ability to apply current theory to practice. Demonstrates organization and critical thinking skills to function effectively in charge. Works with the team and/or individual patient in managing the patient's care, and environment. Demonstrates sound clinical judgment and leadership in improving patient care through the use of nursing process. Identifies nursing diagnosis/clinical problems and related interventions according to Nursing Service/unit policy. Acts as a patient advocate when client self-determination is in question consulting resources such as the ethics committee when appropriate. Complies with Medical Center and Nursing Service ethics policies and standards. Complies with VA Compliance and Business Integrity Program, preventing violations law as well as maintaining high ethical standards. Ensures that self and others comply with standards for patient confidentiality. Maximizes efficient use of staffing resources. Uses creative approaches to pro-actively and safely distribute staff as needed by matching skill mix, skill level, patient acuity/census/workload, and available support services. Utilizes cost containment strategies to effectively manage staffing and material resources. Anticipates potential problems involving resources and/or safety and takes action to correct. Uses knowledge and demonstrates leadership in using resources (services, equipment, and supplies). Ensures safety and wellbeing of self, patients visitors and employees through compliance with work place standards.
