SSM Health Day InstituteSpeech Language Pathologist (SLP)O'Fallon, IL*Per Diem (M-F 8am-4:30pm)*Tier I - $42Tier II - $45SSM Health Day Institute is seeking a Speech Language Pathologist to work in our specialized outpatient rehabilitation program. We have nine centers across the St. Louis Metro area serving individuals recovering from a traumatic injury or illness who no longer require 24-hour nursing or acute rehabilitative care. Per Diem positions are eligible for 401k based on reaching 1,000 hours within their first anniversary or subsequent calendar year. We also offer our employee assistance program to per diem employees.Evaluates assigned patients and establishes appropriate treatment goals and treatment programsProvides ongoing evaluation of patient’s status and goalsDevelops comprehensive goals for patient treatment to maximize outcomes.Coordinates treatment program in conjunction with other clinical/therapeutic servicesAttends appropriate clinical meetings to facilitate coordinationProvides for patient education to include development of comprehensive and progressive home programs with instruction to patient, family and other caregiversPrepares for patient discharge inclusive of ordering of necessary equipment and recommendationsEvaluates patient in actual or simulated home, school, work or community environmentsBachelor’s Degree in Speech Therapy required Current and valid state licensure or eligibility for state licensure as a Speech Language Pathologist requiredCurrent Certificate of Clinical Competence (CCC) in Speech Language Pathology from the American speech-Language & Hearing Association or eligibility for Certification pending professional experience required.
