AFC - Shared Living Case Manager

The Case Manager is primarily responsible for assessing the needs of individuals supported and ensuring placement with appropriate caregivers who have the ability and desire to provide quality care and supports. Ensuring ongoing placements are consistently meeting expectations. Providing advocacy, education and support for individuals and caregivers. The Case Manager may carry a case load up to 15 to 30 AFC members. **Qualifications** A bachelor's degree in social work, a license from the Massachusetts Board of Registration in Social Work, and a minimum of 2 years of related experience working with elderly or disabled adults; or a bachelor's degree and 2 years clinical experience working with the elderly or disabled. Ability to complete all initial and ongoing background checks including but not limited to: CORI, SORI, Fingerprinting, LEIE, Driving record, E-verify, Physical, PPD and licensing and/or certification checks (i.e., nursing).
