JOB SUMMARY: Weare seeking compassionate professionals to join our team as CaseManagers for the Children's Long-Term Support (CLTS) Waiver Program inDane County. As a Case Manager, you will provide service coordinationservices to youth and families needing counseling and referral servicesfor one or more of the following: intellectual and/or developmental,physical, or mental health disabilities. Conductassessments of client needs through interviews, collateral contacts, andother investigations. Maintain an individualized service plan withclients and perform casework services. Maintain a caseload involvingclients with personal, family, or socio-economic problems and attend totheir related protection and safety needs. Counsel and provideintervention and case management services by coordinating varioussocial, community, and other services received by the clients. Completein-person home visits to conduct assessments and maintain clientcontact. SALARY: $24.00-25.00 per hour LOCATION: Madison, WI. Localtravel is required for client home visits throughout Dane County, withmileage reimbursement. SCHEDULE: Monday-Friday 8:00am-5:00pm, withoccasional evenings to accommodate families' schedules.
