St. Bernardine Medical Center is seeking a skilled and detail-oriented Pharmacist to provide comprehensive pharmaceutical care across inpatient services. In this vital role, you will ensure safe medication practices, participate in performance improvement initiatives, and serve as a trusted resource for clinical staff and providers.Key Responsibilities:Review, process, and dispense medications and IV therapies with precision and in compliance with all hospital policies and regulatory guidelinesProvide direct clinical consultation and medication therapy recommendations to physicians and nursing teamsSupervise and support pharmacy technicians in compounding, repackaging, and medication distributionMonitor patient medication profiles for drug interactions, allergies, and contraindicationsPerform pharmacokinetic calculations and adjust dosages as appropriate for patient age, weight, and conditionSupport formulary compliance, therapeutic interchange, and clinical pharmacy initiativesParticipate in department performance improvement projects, medication use evaluations, and adverse drug reaction monitoringEnsure accurate and timely documentation within the EMR and pharmacy systemsMaintain compliance with controlled substance protocols, regulatory standards, and documentation requirementsThis position requires the full understanding and active participation in fulfilling the Mission of St. Bernardine Medical Center. We are looking for candidates who embody our Core Values and are committed to advancing high-quality, safe, and effective patient care.1-2 years experience in Hospital Pharmacy, or completion of a Pharmacy Residency program preferred Experience in unit-dosing, IV admixtures, pharmacokinetics and TPN desireGraduate of a School of Pharmacy, required Pharm D. desirable Basic knowledge of computers desirable California Pharmacist License
